APPOINTMENTS: Please arrive in advance of your scheduled appointment to best accommodate your care when you visit. If you are late more than ten minutes, then you may be rescheduled or incur a fee. If you need to cancel, then we request a phone call (312-321-0004) no later than 2pm the day prior to your scheduled appointment or a cancellation fee will be assessed. Please note your required online intake forms with consents for treatment, directions/parking guidance, and keypad access codes to our buildings are available in your appointment reminder emails - please review in advance of your visit to prepare!
