APPOINTMENTS: Please arrive in advance of your scheduled appointment to support your care when you visit. If you are late more than ten minutes, then you may be rescheduled or incur a fee. If you need to cancel, then we request a phone call (312-321-0004) no later than 2pm the day prior to your scheduled appointment or a cancellation fee will be assessed. Please note your required online intake forms with consents for treatment, directions/parking guidance, and keypad access codes to our buildings are available in your appointment reminder emails - please review in advance of your visit to prepare!
